When you are diagnosed with a chronic medical condition, not only do you need to prepare medically, you need to prepare legally. This session will address important topics such as Powers of Attorney, Trusts, and Wills. What legal options are available in planning and paying for your long-term care? How can Medicare, Medicaid, long-term care insurance, and personal care contracts maximize your quality of life?

Registration for Community Members: There is no charge for community members to attend. However, advanced registration is required. Please call 847-462-0885.

CEUs and Registration: 1.5 Continuing Education Credit will be awarded to Illinois Social Workers, Counselors, Nurses, and Nursing Home Administrators for $10.00. Checks can be mailed to Elderwerks Educational Services, Post Office Box 92, Cary, Illinois 60013.
